Medication Policy and Procedures

Only students who absolutely need to take medications during school hours should be taking medication at school.

Antibiotics prescribed for three times a day should usually be taken at home before school, after school, and at bedtime.

Any student who needs to take medication at school on a temporary or long-term basis will need to complete the following steps:

1. Obtain a Medication Request Form from the Health Office or down load it from the District web site.Click to view, right click to download form.(Mac Users, Control Click link to download form)
2. Have your physician or dentist complete the top portion of the form.
3. Sign, date, and return completed form to the Health Office
Note: Both physician and parent signatures are required.

There have been changes in federal regulations regarding faxing of medical information. Check with your physician’s office about their policy.

A student may carry and self-administer an inhaler prescribed for asthma symptoms providing specific conditions are met. Contact the building nurse for forms and details.

Students are not to carry any other medications in their lunch bags, pockets, or backpacks, except as allowed by District 57 inhaler policy.

You will be responsible for supplying any medication to be administered to your child while at school. This includes Tylenol, Advil, Motrin, cough syrup, cough drops, etc.

All medication must be in its original container.

If an antibiotic is prescribed for an infectious condition, a student must take the medication for a minimum of 24 hours before returning to school.

Only medications authorized by a physician will be administered at school.
