The Illinois Department of Public Health requires a dental examination of all children in kindergarten, second and sixth grades. The authorized form must be completed and submitted to the school health office by May 15th of the child’s kindergarten, second or sixth grade school year. Exams completed up to 18 months prior to the May 15th deadline will be accepted. A hard copy of the Illinois Department of Public Health Dental Examination Form and instructions will be sent home at the start of the 18 month time frame. You may download the dental exam form or request one at any of our school health offices. A Dental Examination Waiver Form is available for students participating in the free and reduced lunch program or Medicaid/KidCare. Please call the nurse for specific information.
